Providing the best care for our new patients means that we need to have some medical history about you. It enables us to create a more personal relationship with you.  It also gives us the opportunity to understand your needs for better and more thorough treatment.

When you first visit our office, we will ask you to fill out some medical forms, and get some dental information about you, too. Once forms are completed, you will receive an individualized consultation with the dentist, who will check on your dental needs and understand the purpose of your visit.

The visit will include x-rays of your teeth for a complete evaluation of your teeth. The dentist will be glad to answer any questions at that time that you have about your treatment.

You can download the forms we require below, including our Privacy Policy. There is also a HIPPA form. Printing them and filling them out in advance will enable you to save time when you come in to our office. They are in PDF format and can be read with the Adobe Acrobat Reader software, which is on most computers.
